Phantom® Intramedullary Nail on Paragon28.com’s Surgeon Finder microsite. My contact information shared with patients will only include my name, hospital affiliation, clinic address/website and phone number.
I acknowledge that when sharing my contact data with patients, Paragon 28 will only consider my training on and/or surgical experience with the TenoTac System as well as proximity to the patient inquiring. For my inclusion on the microsite, Paragon 28 will not consider the volume or value of any previous or future purchases or referrals of their product that I control and I will not expect or request any remuneration from Paragon 28.
I also acknowledge that I have been trained on the Phantom® Intramedullary Nail.
